Our policy

Forms Needed for Enrollment

Here are the most often needed forms for enrollment. You may fill them out ahead of time and have them ready for enrollment or we can provide forms at the time of enrollment.

The "Program Application" must be filled out to enroll. All the information is required by Texas Department of Family and Protective Services.

If your child has severe allergies please have your child's physician sign off on an emergency plan of action.

*All child and family information is handled as confidential.
